{"id":225,"date":"2018-02-18T15:05:07","date_gmt":"2018-02-18T15:05:07","guid":{"rendered":"http:\/\/localhost:8081\/medicalphysics\/?page_id=225"},"modified":"2021-06-25T09:08:25","modified_gmt":"2021-06-25T09:08:25","slug":"radiotherapy-physics","status":"publish","type":"page","link":"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/","title":{"rendered":"Radiotherapy Physics"},"content":{"rendered":"<p>The <strong>Radiotherapy Physics section<\/strong> is responsible for providing scientific services to the <a href=\"https:\/\/www.royalsurrey.nhs.uk\/radiotherapy-\">Radiotherapy Department<\/a> of <a href=\"https:\/\/www.royalsurrey.nhs.uk\/st-lukes-cancer-centre\">St Luke\u2019s Cancer Centre<\/a>, which serves a population of 1.4 million in Surrey and West Sussex and with a satellite facility at East Surrey Hospital.\u00a0 The Centre offers a range of advanced radiotherapy techniques including:<\/p>\n<ul>\n<li><a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/treatment-planning\/\">Intensity modulated radiotherapy<\/a> (IMRT); &gt;90% of radical radiotherapy treatments are delivered using this advanced technique\n<ul>\n<li>For breast radiotherapy, a field-in-field technique is used<\/li>\n<li>For other treatment sites, volumetrically-modulated arc therapy (VMAT) is used<br \/>\n<a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/rapidplan.png\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-medium wp-image-1398\" src=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/rapidplan-300x159.png\" alt=\"\" width=\"300\" height=\"159\" srcset=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/rapidplan-300x159.png 300w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/rapidplan-768x406.png 768w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/rapidplan-1024x542.png 1024w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/rapidplan.png 1280w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a><\/li>\n<\/ul>\n<\/li>\n<li><a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/treatment-planning\/\">Stereotactic Ablative Radiotherapy<\/a> (SABR) for primary lung cancer and oligometastases. The department has been commissioned by NHS England to deliver this service, and was one of three UK centres appointed to provide a mentoring service to help develop SABR nationally.<br \/>\n<a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/sabr-logo.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-1401\" src=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/sabr-logo.jpg\" alt=\"\" width=\"250\" height=\"120\" \/><\/a><\/li>\n<li><a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/research-in-radiotherapy\/\">Motion management<\/a>, including breath-hold techniques to reduce normal tissue toxicity.<\/li>\n<li>A comprehensive brachytherapy service including <a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/research-in-radiotherapy\/image-guided-brachytherapy\/\">image-guided brachytherapy<\/a> (IGBT) treatments and an internationally renowned prostate seed service.<br \/>\n<a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/prostate_brachy.png\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-medium wp-image-1388\" src=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/prostate_brachy-300x284.png\" alt=\"\" width=\"300\" height=\"284\" srcset=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/prostate_brachy-300x284.png 300w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/prostate_brachy.png 500w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a> <a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/venezia.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-medium wp-image-1394\" src=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/venezia-300x225.jpg\" alt=\"\" width=\"300\" height=\"225\" srcset=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/venezia-300x225.jpg 300w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/venezia-768x576.jpg 768w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/venezia-1024x768.jpg 1024w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/venezia.jpg 1200w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a><\/li>\n<li>A<a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/brachytherapy\/\"> contact radiotherapy<\/a> service for rectal cancer<\/li>\n<li>In 2020, the department will be installing a Varian ETHOS Therapy unit, giving the capability for online adaptation of the radiotherapy treatment<br \/>\n<a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/ethos_logo.png\"><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-medium wp-image-1397\" src=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/ethos_logo-300x160.png\" alt=\"\" width=\"300\" height=\"160\" srcset=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/ethos_logo-300x160.png 300w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/ethos_logo-768x409.png 768w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/ethos_logo-1024x546.png 1024w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2020\/02\/ethos_logo.png 1111w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a><\/li>\n<\/ul>\n<p>Excellent links exist with the National Physical Laboratory and a joint consultant physicist post has been appointed to help develop NPL\u2019s clinical services. This has led to <a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/research-in-radiotherapy\/\">national dosimetry intercomparisons<\/a> involving all UK radiotherapy centres for stereotactic radiosurgery (SRS), SABR, VMAT and High Dose Rate brachytherapy demonstrating consistent radiation dose delivery to patients across the UK for these complex treatments. This work has been in collaboration with the National Cancer Research Institute\u2019s (NCRI) Radiotherapy Trials QA group and has also acted as a means of accreditation for clinical trials, thus increasing patient uptake with these advanced radiotherapy techniques.<\/p>\n<p>The section also has links with equipment manufacturers.\u00a0 Through collaboration with Varian Medical Systems, the department performed beta-testing of the knowledge-based radiotherapy treatment planning system, <a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/rapidplan\/\">RapidPlan<\/a>, and delivered the first treatment worldwide planned with this software.\u00a0 As part of an ongoing collaboration with Mirada Medical Ltd, the department are testing <a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/research-in-radiotherapy\/\">auto-contouring software<\/a> designed to streamline the delineation of tumours and normal tissues.<\/p>\n<figure id=\"attachment_1065\" aria-describedby=\"caption-attachment-1065\" style=\"width: 399px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2019\/03\/truebeam-edit.jpg\"><img loading=\"lazy\" decoding=\"async\" class=\"wp-image-1065\" src=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2019\/03\/truebeam-edit-298x300.jpg\" alt=\"\" width=\"399\" height=\"401\" srcset=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2019\/03\/truebeam-edit-298x300.jpg 298w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2019\/03\/truebeam-edit-150x150.jpg 150w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2019\/03\/truebeam-edit-768x773.jpg 768w, https:\/\/medphys.royalsurrey.nhs.uk\/department\/wp-content\/uploads\/2019\/03\/truebeam-edit.jpg 996w\" sizes=\"auto, (max-width: 399px) 100vw, 399px\" \/><\/a><figcaption id=\"caption-attachment-1065\" class=\"wp-caption-text\"><strong>Varian TrueBeam linac equipped with ExacTrak patient position monitoring system for SRS treatments<\/strong><\/figcaption><\/figure>\n<p>The Section is also responsible for ensuring the Trust meets the statutory obligations of the Ionising Radiation Regulations 2017 and the Ionising Radiation (Medical Exposure) Regulations 2017 with regard to radiotherapy, national dosimetry codes of practice and national guidelines.\u00a0 In order to meet this statutory requirement, the team has to work to a high level of expertise and implement national and international standards of dosimetry and safety.\u00a0 The Whole Time Equivalent <a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/people\/\">establishment<\/a> is currently approximately 25 physicists and 10 dosimetrists \/ treatment planners. \u00a0The section is divided into three services:<\/p>\n<ul>\n<li><a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/treatment-planning\/\">Radiotherapy Treatment Planning<\/a><\/li>\n<li><a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/brachytherapy\/\">Brachytherapy Physics<\/a><\/li>\n<li><a href=\"https:\/\/medphys.royalsurrey.nhs.uk\/department\/radiotherapy-physics\/dosimetry-and-quality-assurance\/\">Dosimetry &amp;
